If I'm following you correctly... woman #1 is on her back (missionary), woman # 2 is on top of #1, such that they are belly to belly, #2's knees on the bed straddling #1 (kinda like she's in doggie style from the guys point of view, with a woman under her in missionary... then the guy inserts and removes into slot 1A and then inserts and removes from 2A, then repeats as necessary? In my case prolly about 30 seconds since I would be turned on so much by that time. But, I've shared too much already! :-)

Exactly! If you're gonna blow fast, that's ok...the girls can keep going at it with eachother for a while till you've recovered enough to continue with them.

My wife is one. I found another at a "special" club. and my wife brought in the other. I have had others but they move on...

Gear - its always wise to be extrememly cautious. Be VERY cautious of the windpipe - do not place direct pressure there as it can be injured.

Let her be your guide. Ask her, for example, to make 'mmhmmm' or 'yes' sounds and to guide your squeeze by placing her hand over yours and squeezing and feeling her push back.
