If I'm following you correctly... woman #1 is on her back (missionary), woman # 2 is on top of #1, such that they are belly to belly, #2's knees on the bed straddling #1 (kinda like she's in doggie style from the guys point of view, with a woman under her in missionary... then the guy inserts and removes into slot 1A and then inserts and removes from 2A, then repeats as necessary? Gear - its always wise to be extrememly cautious. Be VERY cautious of the windpipe - do not place direct pressure there as it can be injured.
